Motor neuron disease treatment in India today combines disease-modifying medicines where available with multidisciplinary supportive care that protects mobility, speech, nutrition and breathing. Motor neuron disease (MND), most commonly amyotrophic lateral sclerosis (ALS), is progressive and has no cure. This guide explains the types, the established care, what stem cell research has shown, and how to protect yourself from misleading claims.

What Is Motor Neuron Disease?
MND is a group of conditions in which the nerve cells that control voluntary muscles gradually degenerate, leading to weakness and muscle wasting. Types include amyotrophic lateral sclerosis (ALS), primary lateral sclerosis, progressive muscular atrophy and progressive bulbar palsy. Most cases occur without a known cause, and a minority are inherited.
Symptoms and Diagnosis
- progressive muscle weakness, often starting in a hand, foot or the muscles of speech
- muscle twitching (fasciculations), cramps and wasting
- slurred speech or difficulty swallowing
- later, breathing difficulty
- diagnosis is made by a neurologist using examination, EMG and nerve conduction studies, MRI to exclude other causes, blood tests and, where relevant, genetic testing
Established Care vs Investigational Treatment
| Category | Status | What it involves |
|---|---|---|
| Disease-modifying medicines such as riluzole | Established where prescribed by a neurologist | Modest effect on survival; availability of other medicines varies by country |
| Multidisciplinary supportive care | Standard of care | Physiotherapy, speech and swallowing therapy, nutrition and feeding support, non-invasive ventilation, mobility aids, palliative support |
| Genetic testing and counselling | Standard where indicated | Familial forms, family planning |
| Stem cell therapy for MND | Investigational. Permitted only within approved clinical research | Studies on neuroprotection; no proven benefit |
What Stem Cell Research Shows
Researchers have tested mesenchymal stem cells and other cell types in MND, mainly to protect motor neurons and reduce inflammation. Early studies were encouraging in some measures, but a large controlled trial of one mesenchymal cell approach did not meet its main goal, and no stem cell treatment is approved for MND. Be cautious of clinics that quote improvement rates. Any research participation should be coordinated with your neurologist, and current medicines should not be stopped.
Living Well With MND
- start supportive care early: speech, swallowing and breathing assessments prevent complications
- use mobility and communication aids before they become urgent
- involve family and palliative care teams for planning and comfort
- keep a single neurologist-led team coordinating your care
How Stem Cell Treatment Is Regulated in India
The National Guidelines for Stem Cell Research, 2017 issued by ICMR and the Department of Biotechnology state that any use of stem cells in patients, other than hematopoietic stem cell reconstitution for approved indications, is investigational. In September 2026 the Union Ministry of Health and Family Welfare also issued an advisory on regulation of stem cell therapy reiterating that routine clinical use is limited to officially approved indications.
- Ethics approval: investigational use needs ethics committee and institutional stem cell research committee oversight.
- CDSCO: stem cell-derived products are treated as new drugs, and clinical trials need permission.
- Registration: check any trial on the Clinical Trials Registry – India (CTRI).
- NMC: doctors are expected to avoid advertising unproven treatments as established therapy.
Rules are updated from time to time, so confirm the current position with the official sources listed at the end of this article.
Risks and Safety
Possible effects include pain, swelling, bleeding or infection at the collection or injection site, and in some cases immune reactions. For investigational treatments the larger risk is uncertainty: benefit may be limited or absent, and long-term safety data may be incomplete. Treatment should be done only in a registered medical facility, with written informed consent, and in coordination with your own treating specialist.
Cost Considerations
Costs in India are generally lower than in the USA, UK or Europe, but there is no single price. They depend on the condition, the protocol and cell source, the tests required, the number of sessions, hospital stay, and accommodation and travel. Ask for a written, itemised estimate stating what is included, and avoid large advance payments without documents.
How to Choose a Genuine Clinic
- Ask which approved indication or registered trial covers your condition, and verify it on CTRI.
- Confirm the treating doctors are registered and qualified for your condition.
- Ask about cell source, laboratory standards and quality control.
- Be cautious of claims of “cure”, guaranteed results or quoted success percentages.
- Insist on informed consent documents and a written cost estimate.
- Ask for published data, not only testimonials.
Our patient checklist for choosing a genuine stem cell clinic in India covers this in more detail.
Frequently Asked Questions
Is there a cure for motor neuron disease?
No. Treatment aims to slow progression where possible and manage symptoms.
Is stem cell therapy approved for MND in India?
No. It is investigational and permitted only within approved clinical research.
Should I stop riluzole to try stem cells?
No. Never stop or change medicines without your neurologist’s advice.
Does early supportive care help?
Yes. Early speech, swallowing, nutrition and breathing support improves comfort and safety.
How do I find a registered trial?
Search CTRI or ClinicalTrials.gov and ask your neurologist before joining.
Final Word
Good MND care is led by a neurologist and delivered by a multidisciplinary team. Stem cell research continues, but no approved stem cell treatment exists. Ask for registrations and documents, and keep expectations realistic.
